  • Why am I being billed?
    Patient information is property of the attending physician. Patient has the right to this information but must pay for the cost to reproduce according to New Jersey State Law Act 8:43, A-13.5. A fee of $1.00 per page up to 100 pages not to exceed $100.00 plus actual postage cost. There is also a $10.00 search fee to locate file.
    For patient requests certain fees are waived and a flat fee has been established.

  • Can I pick up my records in person?
    YES - TOG has retained the professional services of Record Reproduction Service to help facilitate the process of Release of Information. They have established an efficient process of getting you your records in a timely fashion. Records can be faxed or mailed to you directly or your medical provider. You may also pick records up at the 1225 Whitehorse-Mercerville  Road, Building D, Suite 220, Mercerville, NJ 08619 office. A Representative will contact you to let you know when they are available for pick up.


  • How long does it take to get my records?
    Record requests can take up to two weeks. Actions of the requester can help shorten the timeline by following instructions and completely filling out forms.

  • Can I have my records sent to my home?
    YES -by completing the form provided by RRS along with the authorization, RRS will forward your records to your home via USPS.


  • Can I get copies of my MRI's or X-rays?
    YES - MRI's  and X-rays are captured in an electronic format and are available on a CD ROM.You may request a copy on CD-ROM that contains embedded viewing software.


  • Can I get a copy of my medical records in an electronic format?
    YES -  Records can be requested in an electronic format for an additional charge.  Records delivered on a CR-ROM in an Adobe PDF format You can download a free copy of the Adobe Viewer by clicking here

  • Can I get multiple copies of my records?
    YES -You can order as many copies as you wish. A fee will be incurred for each copy requested.
